Prom Waiver and Release Form
All people attending the “An Enchanted Forest” – A Homeschool Prom 2025 are required to sign a waiver and release form. Please complete one form for each individual (adults and teens). Only one name should be on each form.
You may print the waiver and release form, sign it, and mail it to:
Homeschool Roster, P.O. Box 614, Bolivar, Tennessee 38008
If you would prefer to e-mail the form(s), it must be in a PDF format. Screenshots, JPG files, etc. will not be accepted. The waiver and release form is due by Thursday, May 1, 2025.

If a form is not received, you will not be able to enter the prom venue.
Tickets may still be purchased for the “An Enchanted Forest” – A Homeschool Prom 2025. Invite your home school friends!
